Participant Waiver and Release of Liability

I hereby give my consent for my child to participate in gymnastics classes, camps, parties, private lessons, and other physical activities at Premier Gymnastics Academy, Inc. I am fully aware that participation in these activities presents a risk of injury. I acknowledge and accept the potential risks and damages that might occur as a result of my child’s participation in such activities.

Nonetheless, I, on my own behalf and on behalf of my child, our heirs, administrators, and executors, do hereby release, indemnify, and agree to hold harmless Premier Gymnastics Academy, Inc., and all persons or entities associated with Premier Gymnastics Academy, Inc., from any and all responsibility or liability for any claims, demands, damages, costs, causes of action, or expenses (including, without limitation, reasonable attorneys’ fees) arising out of or resulting from my child’s participation in gymnastics classes, camps, parties, private lessons, or other related activities — including, without limitation, any personal injury, disability, or property damage incurred or sustained by me or my child during or as a result of these activities.

I understand that our family’s medical insurance policy must cover any medical costs incurred in the event of an accident. I hereby verify that I fully understand and accept the preceding conditions for permitting my child to participate in gymnastics classes, camps, parties, private lessons, and other activities.
